Background
Data from several states have found that people with serious mental illness served by public mental health systems die, on average, at least 25 years earlier than the general population. This fact highlights the importance of the integration of both physical and mental health treatment when serving those with severe mental illnesses. Adult & Child Center received a services grant from the Substance Abuse and Mental Health Services Administration (SAMHSA) to implement an integrated primary and behavioral healthcare program and report on basic outcomes for consumers served. Clients who were enrolled in the program were interviewed at baseline, every 6 months after baseline, and at discharge from service. Blood pressure, height, weight, carbon monoxide levels (smoking screen), and waist circumference were reported along with NOMS interview data at baseline, every 6 months after baseline, and at discharge. HBA1C and/or glucose levels, triglycerides, and cholesterol lab reports were collected and reported to SAMHSA at intake and annually thereafter. Clinical services provided by the community mental health center for the integrated care project included screening, assessment, education, and treatment of co-occurring mental, physical, and substance abuse disorders, as well as group health and wellness education.
